Organ Donor Program
You can request to become an Organ and Tissue Donor simply by filling out the online form below.
Once you have completed the form, press the Submit Request button. The information will be sent
to a qualified DMV associate, who will review your request and update your records. There is no
fee to register as an organ and tissue donor. Your driving record will reflect your new Organ
Donor status. Each time you renew your license you must also renew your decision to register as
an organ donor. The next time you renew your license, your donor designation will be indicated
by a red heart on the front of your license.
This is considered legal consent for organ and tissue donation. This designation may not be revoked
by any person besides the license or ID holder according to Delaware law. Therefore, it is important
to inform your family of your decision to be an organ and tissue donor.
